We respect the many factors and careful thought that go into making decisions for the wellbeing of your loved one. Our team draws on personal and professional experience, working diligently to develop a personalized plan of care for your loved one. Clear Path Home Care offers a unique Total Care Management system to optimize each client’s happiness and wellbeing. A dedicated care manager is assigned to each client to provide maximum accountability. The care manager will be a constant, personal contact for the client and their caregiver. Our Total Care Management system is designed to ensure comfort and peace of mind for our clients and their families. Community Care for Veterans Since 2016, Clear Path Home Care has been contracted with the U.S. Department of Veteran Affairs and Triwest to offer Community Care for veterans. Community Care is offered by the VA and contracted through local home care agencies like Clear Path Home Care to provide supportive medical services to help chronically ill or disabled eligible Veterans of any age remain in their home. These services are referred to as home health care and are considered extended care services prescribed by and under the direction of a VA physician.Please contact Clear Path Home Care if you are a veteran and in need of care in your home with Activities of Daily Living. Our care relationship managers will help guide you through the process of working directly with your VA physician and Veteran Affairs to start in-home care services. A primary residence and auto are not counted as part of the net worth.
