Community Care for Veterans Since 2016, Clear Path Home Care has been contracted with the U.S. Department of Veteran Affairs and Triwest to offer Community Care for veterans. Community Care is offered by the VA and contracted through local home care agencies like Clear Path Home Care to provide supportive medical services to help chronically ill or disabled eligible Veterans of any age remain in their home. These services are referred to as home health care and are considered extended care services prescribed by and under the direction of a VA physician.Please contact Clear Path Home Care if you are a veteran and in need of care in your home with Activities of Daily Living. Our care relationship managers will help guide you through the process of working directly with your VA physician and Veteran Affairs to start in-home care services. VetAssist Program Clear Path Home Care has also partnered with Veterans Home Care, a company that offers the unique VetAssist® Program. A primary residence and auto are not counted as part of the net worth.
